Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist to assess and retrain patients at home, working with physicians and the care team to restore function and independence.

The role requires current OT licensure, CPR certification, and a valid driver's license with reliable transportation. Compensation includes base pay with a pay range, and a TB and driver-safety program in place.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Treatment.
• Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.
• Review/expand retraining programs in line with behavioral goals and rehabilitation program guidelines.
• Consult with other vocational team members to develop and implement vocationally oriented plans.
• Report patient status and progress to the physician and care team.
• Participate in care coordination and discharge planning activities and act as a resource to other health care personnel.
• Design community reintegration activities to aid physical reconditioning and coordinate with the interdisciplinary team.
• Recommend/adapt equipment to improve residence or work environments and participation in rehab.
• Design/train staff and family to carry out retraining programs including activities of daily living and hygiene skills.
• Supervise and evaluate Occupational Therapy Assistant and Home Health Aide performance.
• Document patient observations, interventions and evaluations accurately; submit plans and summaries to supervisors.
