Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to deliver in-home rehabilitation. You will assess patients, design retraining plans, and work with physicians to optimize functional independence in daily activities.

You will coordinate care with the clinical team, supervise aides, and document progress daily. A valid OT license, CPR certification, and reliable transportation are required. This role offers a competitive pay range and comprehensive benefits.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.
• Minimum six months of occupational therapy experience.
• Home Health experience is preferred.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with physician and care team and participate in development/revision of the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, ROM and function.
• Review/expand retraining programs to meet patient goals.
• Design community reintegration activities and coordinate with interdisciplinary team.
• Report patient status and progress to physician and care management staff.
• Participate in care coordination and discharge planning activities and act as a resource to other health care personnel.
• Design/train staff and family members to carry out retraining programs including dressing, feeding, grooming and hygiene skills.
