Matrix Medical Network is seeking skilled Advanced Practice Providers to perform comprehensive in-home health assessments across members' residences, including geriatrics. You will review medical history, medications, and social factors to identify care gaps and improve outcomes.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• Board certification in relevant NP specialty (Adult, Adult Gerontology, Family, etc.).
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid driver’s license for state of residence.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers; follow up as necessary.
• Provide services at the member’s residence, homes or Skilled Nursing Facilities.
