Job summary

Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments for members in their homes or SNFs. Assessments last about one hour and cover health, meds, and risks with thorough documentation.

The role offers FT opportunities with flexible scheduling across states and potential for expansion into full-time roles. Sign-on bonus available and travel required.

Qualifications

• Master’s Degree or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult/Family/Gerontology specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience.
• Travel up to 80-90% of the time, including overnight travel.
• Valid state driver’s license and auto insurance for residence state.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnoses.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on patient education and follow-up.
• Provide services at member homes or Skilled Nursing Facilities.
