Job summary

Matrix Medical Network is seeking a skilled Nurse Practitioner to perform comprehensive in-home health assessments, review medical history, medications and risk factors, and coordinate care with PCPs and care teams.

The role requires active NP licensure, board certification in adult/gerontology or family practice, and the ability to travel up to 80-90% across states. Competitive pay, mileage reimbursement, and benefits are included.

Qualifications

• Master’s Degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• 1 year of NP experience and board certification in Adult, Gerontology, Family or Emergency care.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, education and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Provide patient education and follow-up in home or SNF settings.
• Adhere to Matrix guidelines and ensure clear documentation.
