Job summary

Matrix Medical Network seeks Nurse Practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications and risk factors. This role emphasizes collaboration with PCPs, care managers and families to close care gaps and improve outcomes.

Providers travel extensively, with flexible scheduling and support for licensure, ongoing education and potential full-time opportunities. Competitive pay and mileage reimbursements are offered.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• Board certification in corresponding NP specialty (AANP/AACN/ANCC).
• Current BLS/ACLS/CPR certification.

Responsibilities

• Perform adult/geriatric comprehensive health assessments including medical history, diagnoses, treatment and referrals.
• Administer point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers and caregivers to coordinate care.
• Provide in-home health assessments at member residences or facilities.
