Qualifications

• Master’s prepared NP with licensure in practicing state
• Board certification in NP specialties as listed
• BLS/ACLS/CPR certification current and maintained

Responsibilities

• Perform comprehensive health assessments in patients' homes or facilities
• Review medical history, medications, and social determinants of health
• Coordinate with physicians, case managers, and other care team members
• Provide education and follow-up based on assessment findings
• Document findings per Matrix guidelines and protocols
