Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for members in the field. You will review medical history, medications, and social factors to identify care gaps and improve outcomes.

The role requires NP licensure in the practicing state, current certifications, and ability to travel up to 80–90% with overnight stays. You will collaborate with PCPs and care teams to deliver personalized care at members’ homes.

Qualifications

• Master’s Degree in NP or commensurate experience.
• Current NP licensure in state of practice with prescriptive authority, if required.
• AANP/AACN/ANCC board certification in Adult, Gerontology, Family, or Acute care.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred.
• Ability to travel 80–90% with overnight stays.
• Valid state driver’s license and vehicle insurance.
• Strong computer skills and familiarity with health record software.

Responsibilities

• Perform adult/geriatric assessments including medical history and diagnosis.
• Order and administer point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Coordinate patient education and follow-up with PCPs.
