Job summary

Matrix Medical Network seeks an experienced Nurse Practitioner to perform comprehensive in-home health assessments. Each visit reviews medical history, medications, and social factors to identify care gaps and improve overall health.

Visits include point-of-care testing, collaboration with PCPs and case managers, and travel across states up to 80-90%. A $1,500 sign-on bonus and competitive pay are offered, with flexible scheduling to fit your availability.

Qualifications

• Must hold NP licensure in the practicing state and prescription authority when required.
• Board certification in Adult, Family, or Gerontology specialties is preferred.
• 1 year of NP experience or equivalent demonstrated in home healthcare or clinical settings.
• Willingness to travel up to 80-90% of the time, including overnight stays across states.

Responsibilities

• Conduct adult/geriatric health assessments in patients' homes or facilities.
• Review medical history, medications, and social factors to identify care gaps.
• Administer point-of-care tests and order lab tests as needed.
• Coordinate with PCPs, case managers, and other care team members.
• Provide patient education and follow-up as required.
