Job summary

Matrix Medical Network offers in-home comprehensive health assessments led by advanced practice providers. Role involves visiting members' homes or facilities, reviewing medical histories and medications, and coordinating with a care team to close care gaps.

The position requires NP licensure, prescriptive authority where applicable, and the ability to travel up to 80–90% across the state and possibly neighboring states. Competitive compensation and a $1,500 sign-on bonus are provided.

Qualifications

• Must have NP licensure in state of practice with prescriptive authority or ability to obtain
• Willingness to travel up to 80-90% across the state and possibly neighboring states
• Current BLS, ACLS or CPR certification
• 1 year of NP experience preferred

Responsibilities

• Conduct adult/geriatric assessments including medical history and treatment plans
• Administer point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers and caregivers
• Provide patient education and follow-up
• Offer services at members' residences or facilities
