Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform in-home Comprehensive Health Assessments. Each visit lasts up to one hour and covers medical history, adherence, social factors, and risk review, with testing as needed.

You'll collaborate with physicians and care teams, travel up to significant percentages, and provide care at members' homes or Skilled Nursing Facilities. A Master's NP with state licensure and board certification is required.

Qualifications

• Master’s Degree and NP licensure in state of practice.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties.
• Current BLS/ACLS or CPR certification.
• 1 year NP experience.
• Willing to travel up to 80–90%, including overnight stays.
• Valid driver’s license and auto insurance coverage.
• Strong verbal and written communication skills and computer proficiency.

Responsibilities

• Conduct adult/geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and other personnel.
• Coordinate with PCP on patient education and follow-up as necessary.
• Provide services at the member’s residence, including homes or SNFs.
