Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in‑home health assessments, reviewing health history, medications, and risk factors at the member’s residence. This role supports care gaps closure and improves overall health outcomes.

The ideal candidate holds NP licensure in good standing, board certification in an appropriate specialty, and current BLS/ACLS. Remote nationwide work with flexible scheduling, generous earning potential, and career development

Qualifications

Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
Current NP licensure in state of practice including prescription authority.
AANP, AACN, or ANCC board certification in Adult, Adult Gerontology, Family, Emergency, or Acute Care.
Current BLS, ACLS or CPR certification.
1 year NP experience preferred; new grads encouraged to apply.
Valid state driver’s license.
Automobile insurance coverage for state of residence.

Responsibilities

Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education and referrals.
Administer and order point‑of‑care and lab‑based testing as needed.
Collaborate with physicians, case managers, social workers, and family members.
Coordinate with PCP on patient education and follow up.
Provide services at members’ residences, including homes or Skilled Nursing Facilities.
