Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive health assessments in patients’ homes across assigned states. You will review health history, medications, and risks to identify gaps in care and coordinate with PCPs for follow-up.

The role requires NP licensure, board certification, and ability to travel up to 80–90% with overnight stays. Excellent communication and computer skills are essential.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority or ability to obtain.
• AANP/AACN/ANCC certification in applicable NP specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience.
• Travel up to 80–90% with overnight travel as needed.
• Valid state driver’s license and auto insurance.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnoses, treatment, education, referrals, and documentation.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, families, and caregivers.
• Coordinate with PCP on patient education and follow-up.
