Job summary

Matrix Medical Network seeks a licensed Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications, and risk factors to improve care quality.

You will collaborate with physicians, case managers, and staff, travel within the assigned state, and provide documentation under Matrix guidelines. A Master’s degree in nursing, active NP licensure, and board certification are required; travel up to 80–90% may apply.

Qualifications

• Master’s Degree in Nursing or commensurate experience with NP licensure.
• Current NP licensure in the state of practice and prescriptive authority.
• AANP/AACN/ANCC board certification in appropriate NP specialties.
• Current BLS/ACLS or CPR certification.
• Minimum 1 year NP experience.
• Ability to travel 80–90% including overnight travel.
• Valid state driver’s license and automobile insurance.
• Experience with health record software and strong communication skills.

Responsibilities

• Conduct adult/geriatric in-home assessments including medical history, diagnoses, and care planning.
• Administer point-of-care testing and coordinate with physicians and case managers.
• Collaborate with PCPs and other health professionals to educate patients.
• Provide services at members' residences including homes and Skilled Nursing Facilities.
• Document findings clearly and in a timely manner according to Matrix guidelines.
