Job summary

Matrix Medical Network provides in-home comprehensive health assessments across the United States. As a Nurse Practitioner, you’ll review medical history, medications, and social factors during visits that can last up to an hour, documenting findings per Matrix guidelines.

You will travel extensively to member homes or SNFs, collaborating with physicians, case managers, and care teams to close care gaps and improve outcomes. A strong NP background and active state licensure are required.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in applicable adult care streams.
• BLS, ACLS or CPR certification.
• 1 year NP experience and willingness to travel 80–90%.
• Valid state driver’s license and auto insurance or ability to obtain.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, and education.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence, homes, or SNFs.
