Job summary

Matrix Medical Network seeks a Nurse Practitioner to join its national in-home health assessment team. You will perform comprehensive geriatric assessments at patients' homes or Skilled Nursing Facilities, reviewing medical history, medications, and overall health to close care gaps.

This role offers flexible scheduling and travel across states with competitive pay based on completed assessments. A Master’s degree and NP licensure are required; board certification is preferred.

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult/Adult Gerontology/Family/Specialty.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers as appropriate.
• Provide services at the member’s residence, home or Skilled Nursing Facility.
