Job summary

Matrix Medical Network seeks a licensed Nurse Practitioner to perform in-home Comprehensive Health Assessments across states. You will review medical history, medications, risks, and provide care coordination with PCPs and care managers. Visits may last up to one hour.

Requires NP licensure, ability to travel up to 80-90%, strong communication, and initiative. We offer flexible scheduling, competitive pay per visit, mileage reimbursement, 401(k), and continuing education support.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority as needed.
• AANP/AACN/ANCC board certification in Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care specialties.
• Current BLS/ACLS/CPR certification.
• Able to travel up to 80-90% across the assigned state and possibly neighboring states.
• Valid state driver’s license.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatment, health education, and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers and caregivers.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence (home or Skilled Nursing Facilities).
