Job summary

Matrix Medical Network provides in-home comprehensive health assessments conducted by licensed Nurse Practitioners. During visits, NPs review health history, current medications, and social factors to assess overall health and identify care gaps.

Qualifications

• Master’s degree or commensurate experience with NP licensure required.
• Active NP licensure in state of practice and prescriptive authority as needed.
• Board certification in Adult/Adult Gerontology/Family/Acute Care specialties preferred.
• Current BLS/ACLS or CPR certification.
• Minimum 1 year NP experience in relevant setting.

Responsibilities

• Perform adult/geriatric assessments including medical history, diagnoses, and care planning.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, and other care team members.
• Provide services at the member’s residence, including homes or SNFs.
• Document findings clearly following Matrix guidelines and protocols.
