Job summary

Matrix Medical Network is seeking a licensed Nurse Practitioner to perform comprehensive in-home health assessments. You will evaluate medical history, medications, and living situations to close care gaps and guide treatment plans. Travel up to 80-90% is required, with assessments conducted in members’ homes or SNFs.

The role offers full benefits, on-going education, and support for licensure in neighboring states. A strong KSA in communication and documentation is essential.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty
• Current BLS, ACLS or CPR certification
• 1 year of NP experience
• Able to travel up to 80-90% of the time for extended periods of time to include overnight travel and weekends across the assigned state and possibly neighboring states
• Valid state driver’s license for the state of residence
• Automobile insurance coverage requiring for the state of residence (or ability to obtain)
• Strong computer skills and familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult / geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point of care and lab-based testing as needed
• Work with physicians, case managers, social workers, family members, and other health personnel
• Collaborate with PCP on patient education and follow up
• Provide services at the member’s place of residence (homes or Skilled Nursing Facilities)
