Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive health assessments in members' homes or Skilled Nursing Facilities. You will review medical history, provide health education, and refer to physicians as needed.

Responsibilities include coordinating with care teams, administering tests, and documenting findings per Matrix guidelines. Expect travel up to 80–90% across states, with FT opportunities and benefits.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• NP licensure in state of practice with prescriptive authority if needed.
• AANP/AACN/ANCC board certification in Adult/Adult Gerontology/Family/ Emergency care.
• BLS/ACLS certification.
• 1 year NP experience.

Responsibilities

• Perform adult/geriatric health assessments in home or facility settings.
• Review medical history, diagnoses, and treatment plans with documentation per Matrix guidelines.
• Order/perform point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Travel up to 80–90% of time across states and weekends.
