Job summary

Matrix Medical Network seeks an experienced Nurse Practitioner to perform comprehensive in-home health assessments for adults and seniors. You will review medical history, medications, and social factors to identify care gaps and coordinate with PCPs for follow-up care.

The role requires current NP licensure, state prescriptive authority if needed, and the ability to travel up to 80–90% across the assigned state. Flexible scheduling and ongoing education support are offered.

Qualifications

• Master’s degree in nursing and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in applicable adult-gerontology or family practice.
• BLS, ACLS or CPR certification.

Responsibilities

• Conduct adult/geriatric assessments including medical history and follow-up.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers and family members.
• Provide patient education and follow-up with PCP.
• Deliver services at patients' homes or SNFs when needed.
