Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform comprehensive health assessments in patients’ homes. Each visit can last up to one hour, reviewing medical history, adherence, social factors, and risks to gain insight into overall health.

You will work with PCPs, case managers, and other care team members; benefits include flexible scheduling, competitive pay per assessment ($90–$115), mileage reimbursement, 401(k), and opportunities for licensure expansion.

Qualifications

• Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
• Current NP licensure in state of practice including prescriptive authority or ability to obtain.
• AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
• Current BLS, ACLS or CPR certification.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers and family members.
