Job summary

Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments, focusing on older adults and at-risk patients. Each visit lasts up to one hour and covers medical history, medications, social factors, and risk profiling in the patient’s home or SNF.

Qualifications

• Master’s Degree or commensurate NP experience and licensure.
• Current NP licensure in state of practice with prescriptive authority if needed.
• Board certification in Adult/ACNP/Family/ Emergency NP specialties.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver's license and auto insurance for home visits.
• Strong communication and interpersonal skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, education, referrals, follow-up, and documentation.
• Administer or order point‑of‑care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, family, and care teams.
• Coordinate with PCP on patient education and follow-up.
• Provide services at members’ residence or in SNFs.
