Job summary

Matrix Medical Network in the United States seeks Nurse Practitioners to perform Comprehensive Health Assessments in members' homes. Each visit lasts up to one hour and combines review of medical history, medications, and social determinants of health to identify care gaps.

You will order and administer point-of-care and lab testing, collaborate with physicians and care teams, and provide services at homes or Skilled Nursing Facilities.

Qualifications

• Master’s Degree in NP and active NP licensure.
• Current NP licensure with prescriptive authority in practice state.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric in-home assessments including medical history and diagnosis.
• Administer and order point-of-care and laboratory testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide services at members' residence or SNF and educate patients.
