Job summary

Matrix Medical Network is seeking qualified Nurse Practitioners to conduct Comprehensive Health Assessments in the member's home or Skilled Nursing Facility. Each visit lasts up to an hour and focuses on health status, medication adherence and social determinants of health to close care gaps.

Providers work with physicians, case managers and other clinicians, offering education and follow-up while traveling within the assigned state.

Qualifications

• Master’s Degree or commensurate experience
• NP licensure in state of practice with prescriptive authority
• AANP/AACN/ANCC board certification in relevant NP specialty
• Current BLS, ACLS or CPR certification
• 1 year NP experience
• Willingness to travel up to 80-90% of the time

Responsibilities

• Perform adult/geriatric health assessments at member homes or facilities
• Review medical history, medications, social factors and risk factors
• Order and administer point-of-care and lab testing as needed
• Collaborate with PCPs, case managers and other care team members
• Document findings clearly and follow Matrix guidelines
