Job summary

Matrix Medical Network is seeking qualified Nurse Practitioners to perform Comprehensive Health Assessments in members’ homes or Skilled Nursing Facilities. Visits last up to one hour and include health history review, medication adherence assessment, and risk evaluation.

Providers work with physicians and care teams, offering testing and documentation per Matrix guidelines. Flexible scheduling and competitive per-assessment pay are provided, with potential to transition from PRN to full-time

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice with prescriptive authority
• AANP/AACN/ANCC board certification in Adult, Adult Gerontology Primary Care, Family, Emergency, or Acute Care
• Current BLS, ACLS or CPR certification
• 1 year of NP experience preferred; new grads encouraged to apply
• Valid state driver’s license
• Automobile insurance coverage for residence state
• Strong computer skills and familiarity with employee health/medical record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation according to Matrix guidelines
• Administer and order point-of-care and lab-based testing as needed
• Collaborate with physicians, case managers, social workers, family members, and other healthcare personnel
• Coordinate with PCP on patient education and follow-up
• Provide services at the member’s residence (home or Skilled Nursing Facility)
