Job summary

Matrix Medical Network seeks qualified Nurse Practitioners to perform Comprehensive Health Assessments in members’ homes, typically lasting up to one hour, focusing on medical history, medications, adherence, and risk factors.

You will collaborate with PCPs, case managers, social workers, and family caregivers, with flexible scheduling, mileage reimbursement, and opportunities for full-time roles. We offer FT sign-on bonuses and ongoing education support.

Qualifications

• Must hold NP licensure in state of practice.
• 1 year NP experience preferred, new grads encouraged to apply.
• Valid state driver’s license and automobile insurance as required.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnosis and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCPs, physicians, case managers, social workers, family members and caregivers as appropriate.
• Provide patient education and follow-up with the healthcare team as necessary.
• Provide services at the member’s residence (homes or Skilled Nursing Facilities).
