Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in members' homes nationwide. You’ll review health history, medications, and risk factors, and coordinate with PCPs to close care gaps.

Role offers flexible scheduling, travel to member residences, and a path to full-time opportunities. Certifications required; competitive pay per assessment with state variation.

Qualifikationen

• Master’s degree or commensurate NP experience with licensure.
• Current NP licensure in state of practice with prescriptive authority.
• AANP/AACN/ANCC board certification in NP specialty.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.

Aufgaben

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, referrals and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members and other care team members.
• Coordinate with PCP on patient education and follow-up.
• Provide services at members' residences (homes or Skilled Nursing Facilities).
