Job summary

Matrix Medical Network seeks seasoned Nurse Practitioners to perform comprehensive in-home health assessments for members across the United States. Each visit reviews medical history, medications, and social factors to identify gaps in care, with documentation per Matrix guidelines.

Providers may administer tests, coordinate referrals, and deliver education at the member?s home or SNF, helping improve outcomes while maintaining flexible scheduling, including weekends and evenings.

Qualifications

Master’s degree or commensurate NP experience.
Current NP licensure with prescriptive authority.
AANP, AACN, or ANCC board certification in NP specialty.
Current BLS, ACLS, or CPR certification.
1 year NP experience preferred; new grads encouraged to apply.
Valid state driving license required.
Auto insurance coverage for state of residence.
Strong computer skills and familiarity with health record software.
Excellent verbal and written communication skills.

Responsibilities

Conduct adult/geriatric health assessments including medical history, diagnoses, treatments, and follow-ups.
Administer and order point-of-care and lab testing as needed.
Collaborate with PCPs, case managers, social workers, and care teams.
Provide services at the member’s residence (homes or Skilled Nursing Facilities).
Coordinate patient education and follow-up with PCPs.
