Matrix Medical Network is seeking skilled nurse practitioners to perform comprehensive in-home health assessments. Visits may last up to one hour and include review of medical history, medications, and social determinants to identify gaps in care.

You will order tests as needed and coordinate with PCPs and the care team to support patient education and follow-up. Practitioners work in members' homes or SNFs with flexible scheduling, earning competitive pay based on completed assessments and

Competenze

• Master’s degree or equivalent experience with NP licensure required.
• Current NP licensure in practice state with prescriptive authority or ability to obtain.
• Board certification in Adult, Adult Gerontology Primary Care, Family, or Emergency/Acute Care.
• Strong communication and documentation skills.

Mansioni

• Conduct adult/geriatric health assessments during home visits, documenting history and findings per Matrix guidelines.
• Administer point-of-care tests and coordinate referrals with PCPs and care team.
• Collaborate with physicians, case managers, and family members to support patient education and follow-up.
• Provide care at the member's home or in a Skilled Nursing Facility.
