Job summary

Matrix Medical Network is expanding its national in-home health assessment program. Providers perform comprehensive assessments at members’ homes, reviewing medical history, medications, and social factors to identify care gaps and improve overall health outcomes.

This role offers flexible scheduling, including weekends, with competitive pay based on the number of assessments completed. We provide onboarding, ongoing education, and opportunities to transition to full-time roles.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology or Family NP specialties.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver's license and auto insurance coverage.
• Strong computer skills and familiarity with health record software.

Responsibilities

• Conduct adult/geriatric assessments including history, diagnosis, treatment, and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and families as appropriate.
• Provide in-home services at members’ residences, including SNF settings as needed.
• Coordinate patient education and follow-up with PCPs.
