Job summary

Matrix Medical Network seeks qualified NPs to perform Comprehensive Health Assessments in members’ homes, collaborating with PCPs and care teams. Each visit can last up to one hour and focuses on medical history, medications, adherence, and social factors to identify care gaps.

Providers administer tests, coordinate referrals, and document findings per Matrix guidelines. Flexible scheduling includes weekends, with opportunities for full-time roles and state licensure support.

Qualifications

• Master’s degree and NP licensure required.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care specialties preferred.
• Active NP license and prescriptive authority as needed.

Responsibilities

• Conduct adult/geriatric health assessments and document findings per Matrix guidelines.
• Administer point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers, and family caregivers.
• Provide in-home health assessments at members’ residences, including SNFs when applicable.
