Job summary

Matrix Medical Network is seeking skilled Nurse Practitioners to perform in-home comprehensive health assessments, reviewing medical history, medications, and social factors to close care gaps. Visits occur at the member’s home or in Skilled Nursing Facilities, with emphasis on patient education and follow-up.

We offer flexible scheduling across the nation and competitive compensation. Training and licensure support are provided, with potential for full-time roles and state licensure assistance.

Qualifications

• Must hold NP licensure and state-specific prescriptive authority or the ability to obtain.
• Board certification in Adult/Adult-Gerontology/Family/Acute Care specialties.
• Current BLS, ACLS or CPR certification.
• Minimum 1 year NP experience; new grads encouraged.
• Valid driver’s license and auto insurance for residence state.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and documentation.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and caregivers.
• Provide services at the member’s home or Skilled Nursing Facilities.
