Job summary

Qualifications

• Active, unencumbered nursing license in the assigned state (Louisiana).
• ANCC/AANP/AACN Nurse Practitioner Certification as Adult/Gerontic/Family/ACNP.
• Prescriptive authority in Louisiana or ability to obtain post-hire.

Responsibilities

• Conduct clinical assessments for health plan members.
• Document and code encounters in EMR.
• Identify diagnoses for care management and inform PCPs of gaps in care.
• Educate members on diseases, medications, and compliance.
• Initiate referrals and ensure HIPAA compliance.
• Work autonomously, travel to member sites as needed.
