Job summary

UnitedHealth Group is seeking an Advanced Practice Clinician to join the Senior Community Care program and support high-risk plan members in long-term, nursing and home care settings. The role requires travel across PA, DE, NC, SC and GA, with 85% travel and reimbursement of expenses; licensure in PA/NC/SC is required.

You will perform comprehensive assessments, coordinate care with an interdisciplinary team, provide counseling and education, and document thoroughly using EMR systems.

Qualifications

• Active ANCC or AANP certification for NP; or NCCPA certification for PA

Responsibilities

• Conduct comprehensive assessments including history, vitals, and physical examination
• Support members during short-term sub-acute rehabilitation episodes
• Ensure accurate and complete documentation (ICD-10)
• Collaborate with interdisciplinary care team
• Conduct advanced illness and advanced care planning conversations
• Provide counseling and education to patients and caregivers
