Job summary

UnitedHealth Group is seeking a Senior Advanced Practice Clinician to provide care in nursing homes, senior housing, and related settings across WA, OR, AZ, UT, CO. 85% travel with expenses covered; base salary plus incentives and comprehensive benefits.

The role requires NP/ANCC/AANP certification, active licensure, and DEA where applicable. You will coordinate care and counsel patients and families in diverse environments.

Qualifications

• Active NP/APN certification (ANCC or AANP).
• Current, unrestricted NP licensure in practicing state.
• DEA licensure or prescriptive authority where required by state law.

Responsibilities

• Perform comprehensive assessments including history, vitals, and exams.
• Support sub-acute rehabilitation episodes and coordinate care.
• Document accurately with ICD-10 coding and care plans.
• Collaborate with interdisciplinary care team across settings.
• Discuss advanced illness and end-of-life planning with patients and families.
