Job summary

Matrix Medical Network seeks a qualified Nurse Practitioner to perform Comprehensive Health Assessments in patients’ homes or Skilled Nursing Facilities. You will review medical history, medications, and risk factors during visits that last up to one hour, coordinating with PCPs and care teams to close care gaps.

Candidates must hold NP licensure with prescriptive authority and be comfortable traveling to members’ residences; strong communication and documentation skills are essential.

Qualifications

• Master’s Degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority (or ability to obtain).
• AANP/AACN/ANCC board certification in relevant NP specialty.

Responsibilities

• Conduct adult/geriatric assessments including medical history and diagnosis.
• Administer point-of-care and lab-based testing when needed.
• Collaborate with physicians, case managers, social workers, and family members.
• Provide services at members' residences (home or SNF).
