UnitedHealth Group is seeking a Senior Advanced Practice Clinician to deliver comprehensive care to high-risk members in home and sub-acute settings. The role emphasizes accurate assessments, clinical documentation, and collaboration with the care team to support complex geriatric patients.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• Active, unrestricted NP licensure in the practicing state or ability to obtain by start date
• Active and unrestricted driver's license and access to reliable transportation
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
• Ability to gain a collaborative practice agreement, if applicable in state
• Ability to navigate varied environments; sit/stand/kneel as needed

Responsibilities

• Conduct comprehensive assessments including history, meds, vitals and exam
• Support members during short-term sub-acute rehab episodes
• Ensure documentation including ICD-10 conditions
• Collaborate with interdisciplinary care team
• Discuss advanced illness and end-of-life care planning with patients
• Provide counseling and education to patients and caregivers
