Job summary

Optum Home & Community Care is hiring for an NP/PA to provide in-home primary care. You will assess patients, manage acute and sub-acute episodes, and coordinate with an interdisciplinary team to deliver holistic care in the home.

This role emphasizes compassionate counseling, detailed documentation, and adaptive practice across varied home environments. Travel daily within the assigned area is required.

Qualifications

• Active certification in family, adult, geriatric, or adult-gerontology primary care.
• Current state licensure without disciplinary actions; DEA licensure if applicable.
• Access to reliable transportation and a valid driver's license.

Responsibilities

• Perform comprehensive in-home assessments and medical history reviews.
• Manage acute events and chronic condition exacerbations in patients' homes.
• Document thoroughly, including ICD-10 codes and care plans.
• Collaborate with interdisciplinary care teams and conduct end-of-life discussions.

Skills

Active ANCC or AANP certification (F/S

Active, unrestricted driver's license

DEA licensure / prescriptive authority

Ability to travel to patient homes

Education

Nurse Practitioner or Physician Assistant degree
