UnitedHealth Group is hiring a Nurse Practitioner to conduct clinical assessments for PH members, document in EMR, and coordinate care with PCPs. The role requires travel to member homes and clinics, evaluating vitals, and identifying diagnoses for optimized health outcomes.

Required are an active NP license in LA, certification from ANCC/AANP/AACN, and prescriptive authority or ability to obtain it post-hire. A BLS certification and clean driving license are needed.

Competenze

• Active, unencumbered license to practice nursing in the assigned state.
• 1+ years of clinical experience in highest level of education.
• Active Nurse Practitioner Certification from ANCC, AANP or AACN as an Adult, Geriatric, Family or Acute Care NP.
• Active or ability to obtain prescriptive authority post-hire in Louisiana.

Mansioni

• Conduct assessments in member homes and clinics with emphasis on documentation.
• Check vitals and perform physical exams including foot exams.
• Identify diagnoses for care management and active medical management.
• Communicate assessment findings to inform PCP of gaps in care.
• Educate members on diseases, medications, and compliance.
• Initiate referrals and ensure HIPAA compliance.
• Travel to member sites and work autonomously in varied environments.
