Job summary

Matrix Medical Network is seeking experienced Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes across the region. Each visit lasts up to one hour and includes review of medical history, medications, and living environment to identify care gaps.

Providers administer tests as needed, collaborate with PCPs and care teams, and deliver care in homes or SNFs. We offer flexible scheduling, competitive pay based on completed assessments, and opportunities for state

Qualifications

• Master’s degree or commensurate NP experience.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology, Family or Emergency NP specialties (AANP/AACN/ANCC).
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses, treatment, education, referrals, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, and care staff.
• Coordinate with PCP on patient education and follow-up.
• Provide services at the member’s residence, homes or Skilled Nursing Facilities.
