Matrix Medical Network is seeking Nurse Practitioners to perform in-home comprehensive health assessments. You will review health histories, administer point-of-care tests, and coordinate with physicians and care teams. Assessments occur at members' homes or SNFs, with flexible scheduling throughout the week.

Ideal candidates hold NP licensure, board certification in an NP specialty, and BLS/ACLS. New grads are encouraged to apply and opportunities for professional development are offered.

Qualifikationen

• Must hold NP licensure in the state of practice.
• Board certification in Adult/Family/GERO or related NP specialty.
• BLS, ACLS or CPR certification for clinical practice.
• 1 year NP experience preferred; new grads encouraged to apply.

Aufgaben

• Conduct adult/geriatric assessments including history, diagnosis and treatment planning.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide clinical services at the member's home or Skilled Nursing Facility.
• Coordinate education and follow-up with PCPs.
