Matrix Medical Network seeks experienced Nurse Practitioners to perform in-home Comprehensive Health Assessments. Visits cover medical history, medications, and social factors, with care gaps closed in home or SNFs. You’ll collaborate with physicians and care teams to improve patient outcomes.

Competenze

• Master’s degree in nursing or equivalent; NP licensure required.
• Current NP licensure in state of practice with prescribing authority.
• Board certification in Adult, Adult-Gerontology, Family, or related NP specialty.
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Mansioni

• Conduct adult/geriatric assessments including history, diagnosis, treatment, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with physicians, case managers, social workers, and family members for coordinated care.
• Provide services at the member’s home or Skilled Nursing Facility.
• Support patient education and follow-up with PCP.
