Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in patients’ homes across the country.

Each visit lasts up to an hour and covers health status, medical history, medication adherence, and social factors. You’ll coordinate with PCPs, order tests as needed, and work from homes or skilled facilities with flexible scheduling and competitive pay.

Formación

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice and prescriptive authority as needed
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care specialties
• Current BLS/ACLS or CPR
• 1 year NP experience preferred; new grads welcome

Responsabilidades

• Conduct in-home Comprehensive Health Assessments including medical history and physical status
• Assess medication adherence, social environment, and other health risks
• Coordinate with Primary Care Physician for education and follow-up
• Order point-of-care and lab testing as needed
• Provide care at the member’s residence or in facilities
