Matrix Medical Network is seeking a Nurse Practitioner to perform comprehensive in-home health assessments across the nation. You will review medical history, medication adherence, and social factors to identify care gaps and improve outcomes.

Responsibilities include conducting geriatric assessments, coordinating with PCPs, and offering education. The role supports flexible scheduling, with opportunities to transition to full-time roles and a competitive pay structure.

Competenze

• Master’s Degree in nursing with NP licensure and satisfactory completion.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care as applicable.
• Current BLS/ACLS/CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid driver's license and auto insurance for home visits.

Mansioni

• Perform adult/geriatric health assessments including medical history and diagnoses.
• Administer and order point-of-care and lab-based tests as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Provide in-home services at members' residences or facilities.
