Job summary

Matrix Medical Network seeks a Licensed Nurse Practitioner to perform Comprehensive Health Assessments during in-home visits, reviewing medical history, medications adherence, social factors, and other risks to identify care gaps and improve outcomes.

Providers collaborate with physicians, case managers, social workers, and families, visiting members at their residence or SNFs with flexible scheduling and potential FT opportunities, supported by onboarding, training, and ongoing professional

Qualifications

• Must hold current NP licensure in the state of practice and prescriptive authority as needed.
• Board certification in Adult, Gerontology, Family, or Emergency NP specialties is required.
• Current BLS/ACLS or CPR certification is required.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct Comprehensive Health Assessments (CHA) during member home visits, reviewing health history, meds, and social determinants.
• Administer point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and family members to coordinate care.
• Provide patient education and follow-up in partnership with PCPs.
