Job summary

Matrix Medical Network seeks licensed NPs to perform Comprehensive Health Assessments in members' homes or SNFs, reviewing medical history, medications, adherence, and risk factors during visits that last up to one hour.

You will collaborate with PCPs, case managers, and other care team members, receive a $1,500 sign-on bonus, flexible scheduling, and competitive pay based on assessments completed.

Qualifications

• Master’s Degree or commensurate experience and NP licensure
• Current NP licensure in state of practice including prescriptive authority
• AANP, AACN, or ANCC board certification in one of the listed specialties
• Current BLS, ACLS or CPR certification
• 1 year NP experience preferred; new grads encouraged to apply
• Valid state driver's license
• Automobile insurance coverage for state of residence
• Strong computer skills and familiarity with health record software
• Excellent verbal and written communication skills

Responsibilities

• Conduct adult/geriatric health assessments with history, diagnosis, treatment and documentation
• Administer point-of-care and lab testing as needed
• Collaborate with physicians, case managers, social workers, family members, and other staff
• Educate patients and perform follow up with PCP as necessary
• Provide services at patients' homes or SNFs
