Job summary

Matrix Medical Network is seeking compassionate Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes, often in geriatric settings. Each visit lasts up to one hour and blends review of medical history, current health status, adherence, and social factors to identify gaps in care.

You will collaborate with physicians and care teams, order tests as needed, and work across home and facility settings with flexible scheduling, some weekend hours, and a generous sign-on

Qualifications

• Master’s degree required or commensurate NP experience.
• Current NP licensure in state of practice with prescriptive authority as needed.
• BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct full adult/geriatric health assessments in patient homes or facilities.
• Review medical history, current medications and adherence; identify care gaps.
• Order and administer point-of-care and lab tests as needed.
• Coordinate with physicians, case managers, social workers and caregivers.
• Document findings and provide patient education and follow-up.
