Job summary

Matrix Medical Network delivers in-home comprehensive health assessments nationwide, enabling clinicians to work flexible hours and meet patients where they live. Providers review medical history, medication adherence, and social context to capture a fuller health picture during each one-hour visit.

Qualifications

• Master’s degree in nursing or commensurate NP experience with licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult/Family/Gerontology or related NP specialty (AANP/AACN/ANCC).
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.
• Valid state driver’s license and auto insurance.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnosis, treatment, education, referrals, follow-up, and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, families, and other care team members.
• Coordinate with PCP on patient education and post-visit follow-up.
• Provide services at the member’s residence, including homes or SNFs.
