Matrix Medical Network seeks Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes across the US. Visits can last up to an hour, reviewing health history, medications, adherence, and social factors to identify gaps in care.

Responsibilities include adult/geriatric assessments, point-of-care testing, and collaboration with PCPs and care teams. Flexible scheduling and a $1,500 sign-on bonus are offered, with competitive pay and travel support.

Qualificações

• Master’s degree in nursing or commensurate NP experience.
• Current NP licensure in state of practice and prescriptive authority.
• Board certification in a relevant NP specialty (e.g., Adult, Family).
• Current BLS/ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver license and auto insurance.
• Familiarity with health/medical record software.

Responsabilidades

• Conduct adult/geriatric assessments including history and diagnostics.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers, and staff.
• Coordinate patient education and follow-up with PCP.
• Provide services at patients’ homes or designated facilities.

Conhecimentos

Strong computer skills

Excellent verbal and written

Formação académica

Master’s Degree in Nursing or commensurate NP experience

Current NP licensure in state of practice

Board certification in Adult/Family/AGNP NP specialties

Current BLS/ACLS or CPR certification

1 year NP experience preferred; new grads encouraged

Valid state driver’s license
